Libby Bergman PT, DPT, OCS, FAAOMPT, MTC specializes in the treatment of Orthopaedic dysfunctions of the spine and extremities. Libby is a graduate of the University of Wisconsin-Madison and completed her Doctor of Physical Therapy at the University of Saint Augustine in 2007. She is a Board Certified Orthopaedic Clinical Specialist, (OCS), a fellow of the American Academy of Orthopaedic Manual Physical Therapy (FAAOMPT) and holds a certification in Manual Therapy (MTC) from the University of Saint Augustine.

Libby is an active APTA member currently serving on the Orthopaedic Section Practice Committee and the Credentialing Services Committee for the American Board of Physical Therapy Residency and Fellowship Education. She recently completed a faculty appointment at the University of Saint Augustine where she developed curriculum and mentored clinicians in the Orthopaedic Residency and Orthopaedic Manual Physical Therapy Fellowship programs. She is currently continuing education faculty for the University of Saint Augustine teaching the E2: Extremity Integration seminar nationally and internationally.

Libby is an active triathlete who has competed in four world championship events including the Ironman World Championships in Kona, Hawaii and the International Triathlon Union Age Group World Championships in Auckland, New Zealand. Libby enjoys spending time with her husband Neiland and two young boys Curren and Naish. She is eager to explore the Flathead region with her family and take up trail running, mountain biking, hiking and skiing after her recent move from Saint Augustine, Florida.

Zach Cupples, PT, DPT, CSCS, PRC, OCS

[text_block style=”style_1.png” align=”left”]Dr. Zac Cupples, is a Physical Therapist and Strength Coach who studied under the nationally-recognized Bill Hartman. He graduated from the St. Ambrose University Physical Therapy Program in 2011, and completed an orthopedic residency in 2013. He is currently the Director of the Physical Therapy Program at East Valley Spine and Sports Medicine Center in Chandler, AZ.

Zac has much experience working with the chronic pain, industrial, and athletic populations. His treatment process has a neuroscience foundation through principles taught by the Postural Restoration Institute and modern pain science. His ultimate goal is to give people that work with him independence.

Zac has formal training in various manual therapy to include Maitland, Mulligan, dermoneuromodulation, PRRT, and IASTM. He also has training in the Functional Movement Screen (FMS)/Selective Functional Movement Assessment (SFMA), Graded Motor Imagery, and DNS according to Kolar.

Zac is currently a physical therapist and strength and conditioning coach for the Memphis Grizzlies.[/text_block]

Brian Bochette, PT, DPT, CSCS, USAW,

[text_block style=”style_1.png” align=”left”]Dr. Bochette has been serving as an adjunct professor for Florida Gulf Coast University since 2010.  He holds a Bachelor of Science in Human Performance from Florida Gulf Coast University, a Master of Science in Physical Therapy from Florida Gulf Coast University, and a Clinical Doctorate in Physical Therapy from the University of South Florida. He is a Florida licensed Physical Therapist as well as a Certified Strength and Conditioning Specialist through the National Strength and Conditioning Association. He currently practices outpatient physical therapy and in addition to offering performance based training services.[/text_block]

Eric Chaconas, PT, DPT, CSCS, MTC, PhD, FAAOMT

[text_block style=”style_1.png” align=”left”]Dr. Eric Chaconas became the assistant program director for the Department of Physical Therapy at the university in 2014. He is also an assistant professor in the Doctor of Physical Therapy (DPT) program, where he has served since 2008.

Prior to joining the university, he worked as a physical therapist in both the outpatient orthopaedic and home health settings. He is a member of the American Physical Therapy Association (APTA), as well as its Florida chapter (the FPTA), the American Academy of Orthopaedic Manual Physical Therapists (AAOMPT), and the National Strength and Conditioning Association (NSCA). He serves as vice speaker of the FPTA’s Assembly of Representatives’ Board of Directors, and formerly served as chair of the FPTA’s Government Affairs Committee and as a FPTA federal affairs liaison.

Dr. Chaconas also regularly volunteers. He is currently on the Good Samaritan Health Centers’ Wildflower Clinic Board of Directors and volunteers at the Pink up the Pace breast cancer fundraising organization. He has also served as a volunteer physical therapist for the Good Samaritan Health Centers’ free medical clinic.

He was honored with the FPTA’s Rick Shutes Committee Service Award in 2011 and the APTA’s Emerging Leader Award in 2010. He is a 2008 AAOMPT Fellow, completed a Manual Therapy Certification (MTC) through the university in 2006, and an Orthopaedic Manual Physical Therapy Fellowship in 2008. Dr. Chaconas is currently engaged in both research and clinical practice with an emphasis on orthopaedic and sports rehabilitation.[/text_block]

Tom Eberle, PT, DPT, FAAOMT

[text_block style=”style_1.png” align=”left”]Dr. Eberle is a physical therapist who has served the south Florida community since becoming a licensed physical therapist in 1998, graduating from D’Youville College in his hometown Buffalo, New York. Dr. Eberle continued his studies, completing a Doctorate of Physical Therapy in 2000 with the Ola Grimsby Institute in FT. Lauderdale and then an Orthopedic Fellowship program for the American Academy of Orthopedic Manual Physical Therapy (AAOMPT) through the Ola Grimsby Institute in 2006. Further Dr. Eberle is Kinematic Taping Concept Certified (KTCC) and is one of three of the American faculty in the International Kinematic Taping Association (IKTA). Dr. Eberle currently treats patients at the Holy Cross Orthopedic Institute in Ft. Lauderdale, Florida. Dr. Eberle is an adjunct professor at Florida International University, teaching orthopedic physical therapy. He has formulated an eclectic, evidence based manual therapy approach to treating patients which is shared through the curriculum of the various programs offered by his continuing education company, the Florida Institute of Orthopedic Manual Physical Therapy (FIOMPT).  FIOMPT provides post graduate orthopedic physical therapy continuing education and orthopedic certification programs.  Dr. Eberle was elected to the Florida Physical Therapy Association Board of Directors in 2013 and appointed to the Florida Automobile Insurance Fraud Strike Force in 2014. Dr. Eberle has published an orthopedic manual for the evaluation and treatment of orthopedic pathologies, is published in several peer reviewed medical journals, and presents research and educational seminars nationally and internationally. In October 2012, Dr. Eberle attended and presented research on the effects of manual therapy and exercise at the International Federation of Orthopedic Manual Physical Therapists (IFOMPT) in Quebec City, Ontario. In 2013 he was the key note speaker at an orthopedic conference hosted by the Sedes Sapientiae Catholic University in Lima, Peru. Dr. Eberle is a member of the American Physical Therapy Association and an active Fellow in AAOMPT.   Dr. Eberle resides in Ft. Lauderdale, Florida with his wife who also is a doctor of physical therapy. They enjoy travelling, cooking, and the arts, being at the beach, and staying active.[/text_block]

Logan Sullivan, PT, DPT, EXS

Steve Fell, PT, DPT, EXS, MTC, SCS

[text_block style=”style_1.png” align=”left”]Steve was appointed as the Assistant Fitness coach of the Houston Dynamos first team following a season as the Head Fitness coach for the Rio Grande Valley Toros Football Club, the USL affiliate of the Houston Dynamo.  

Prior to accepting the role with the RGV FC Toros, Steve was the Director of Sports Performance for Flagler College with the role of ensuring physical readiness for 14 varsity sports.  Steve has also worked as an outpatient physical therapist where he was responsible for the evaluation and treatment of orthopedic conditions with a focus on manual therapy.

Steve graduated from Towson University in 2005 with a degree in Exercise Science.  While at Towson, Steve was a four year member of the Men’s soccer team where he later went on to play three seasons with the Baltimore Blast of the Major Indoor Soccer League.  Afterward, Steve attended the University of Saint Augustine for Health Sciences where he obtained his Doctorate in Physiotherapy in 2012.  Additionally, in 2015, Steve became a board certified sports physiotherapist and certified EXOS Performance Specialist.[/text_block]

Adam Wolf, PT, LMT, FAFS,

[text_block style=”style_1.png” align=”left”]Adam is a Physical Therapist and Massage Therapist specializing in Applied Bio mechanics.

Adam’s professional credentials include Licensure in Physical Therapy (IL) and Massage Therapy (IL), Fellow of Applied Functional Science (Gray Institute), Level III Neurokinetic Therapy (NKT) practitioner, Enhance Running Technician, Functional Range Conditioning (FRC) practitioner, and has been an Elite Provider of Active Release Technique (ART), . John Snyder, PT, DPT, CSCS

[text_block style=”style_1.png” align=”left”]Dr. Snyder received his Doctor of Physical Therapy degree from the University of Pittsburgh. While enrolled in graduate school, he served as the Vice President of the American Academy of Orthopaedic Manual Physical Therapists (AAOMPT) Student Special Interest Group. He also created and frequently contributed to OrthopedicManualPT.com, which was a blog devoted to evidence-based management of orthopedic conditions. This blog, which has now been moved to the Snyder Physical Therapy & Wellness website, had the honor of being voted ‘Best Student Blog’ during Therapydia’s Annual PT Blog Awards in 2013 and 2014. Outside of his didactic and clinical education, Dr. Snyder also worked as personal trainer at Greater Pittsburgh Wellness in Fox Chapel, Pennsylvania. Prior to beginning graduate school, he earned his Bachelor of Science in Applied Sciences degree from Youngstown State Universitywhere he studied Exercise Science and played ACHA Division 1 Ice Hockey.

Currently, Dr. Snyder specializes in the treatment of an athletic population with emphasis on hip, knee, and overhead throwing injuries. The rehabilitation of the injured athlete is different from the typical patient and requires a completely different treatment approach with emphasis on movement re-education and functional testing in order to determine the patient’s readiness to return to their chosen sport and prevent re-injury. Additionally, he has great deal of interest in manual and manipulative therapy and its role in the treatment of orthopedic and spinal conditions and is currently working towards earning the Certified Orthopedic Manual Therapist (COMT) designation through Maitland-Australian Physiotherapy Seminars in order to further his knowledge and expertise in this area.[/text_block]
